We have hardcopy forms at our front desk. You can stop by and fill them out there.
YOU MUST COMPLETE THE HARDCOPY FORMS IN STEP 2 TO BE CONSIDERED AS FULLY REGISTERED ALONG WITH COMPLETING THE ONLINE REGISTRATION IN STEP 1.
1. Register online and complete this step with payment of the registration fee. The waitlist option will not require a payment until you are moved to the active list.
2. You must fill out our child information form (per child), sunscreen/medicine information form (per child), and a draft agreement form. You can email the completed documents to callie@athensmcminnymca.org or drop it off to the front desk.

4. The childcare team will reach out to you after completing these steps. If you do not complete these steps, you will not be considered as fully registered for 2024 Summer Day Camp. For questions, please email the team at callie@athensmcminnymca.org.
